The shock hits, your breathing goes ragged, and for about thirty seconds you wonder why on earth you do this to yourself. Then something shifts. Your breathing steadies. A warmth starts to build from the inside. And by the time you climb out — pink-skinned, slightly euphoric, shaking hands that do not quite work properly — you feel more alive than you have in weeks.
This is cold water therapy. And while it has become fashionable in recent years, thanks largely to Wim Hof and a parade of wellness influencers, the science behind it is genuinely compelling. More interesting still is what it does to your relationships. Cold water swimming is one of the fastest ways to build real, lasting friendships — and Devon happens to be one of the best places in the country to do it.
What Happens to Your Body in Cold Water
The Vagus Nerve Response
When cold water hits your skin — particularly around the face, neck, and chest — it activates the vagus nerve, the longest cranial nerve in your body. The vagus nerve runs from the top of your neck down both sides, into your chest, and through your abdomen, and it plays a central role in regulating your parasympathetic nervous system — the part responsible for calming you down.
Cold water stimulation slows your heart rate and redirects oxygen to vital organs. It is, in essence, your body's built-in reset button. Regular stimulation of the vagus nerve has been linked to improved mood regulation, reduced anxiety, and a greater sense of emotional balance. This is not fringe science — vagus nerve stimulation is used as a clinical treatment for depression and epilepsy.
The Endorphin and Dopamine Flood
Cold immersion triggers a cascade of neurotransmitters. Research has shown that cold water exposure can increase dopamine production by up to 250 per cent, while norepinephrine — associated with mood regulation, attention, and stress response — can surge by as much as 530 per cent. Endorphins, the body's natural painkillers, flood the system simultaneously.
This is not a subtle effect. Anyone who has stood on a Devon beach after a winter swim, grinning at nothing in particular, knows exactly what this chemical cocktail feels like. It is the reason swimmers describe feeling "buzzed" or "high" for hours after getting out.
Cold water immersion has been shown to increase dopamine by up to 250 per cent and norepinephrine by up to 530 per cent. These neurotransmitter surges are associated with improved mood, sharper focus, and reduced symptoms of depression and anxiety.
Inflammation and Stress Adaptation
Cold exposure causes vasoconstriction — the narrowing of blood vessels — which reduces local blood flow and decreases the accumulation of inflammatory mediators. While the long-term anti-inflammatory effects are still being studied (the research is promising but mixed), there is strong evidence that regular cold water immersion helps the body adapt to controlled stress.
A 2024 study found that short-term cold exposure may actually train the nervous system to handle stress more effectively. This is the principle of hormesis — small, controlled doses of stress making the body more resilient over time. Swimmers who stick with it often find that they become calmer, more grounded, and better able to cope with the everyday stresses that used to rattle them.
The Research: From Portsmouth to the BBC
Dr Mark Harper and the Clinical Case
Much of the serious UK research into cold water swimming and mental health has been driven by Dr Mark Harper, a consultant anaesthetist at Brighton and Sussex University Hospitals. His professional background is in keeping surgical patients warm, but his personal passion lies in the opposite direction.
In collaboration with Professor Mike Tipton's Extreme Environments Laboratory at the University of Portsmouth, Dr Harper has been at the forefront of testing whether cold water swimming can work as a clinical intervention for mental health conditions. His work gained national attention through the BBC television programme "The Doctor Who Gave Up Drugs" (which went on to receive a BAFTA nomination), where cold water swimming was tested as a treatment for depression.
Subsequently, Dr Harper was part of the team that ran the first clinical trial using sea swimming as an intervention for anxiety and depression. The results were striking enough to warrant his 2022 book, Chill: The Cold Water Swim Cure, which lays out both the physiological mechanisms and the growing body of evidence.
A 2025 exploratory study published in Lifestyle Medicine examined the effects of regular cold-water sea swimming on daily indices of mental health, adding to the growing evidence base. Meanwhile, a study involving undergraduate students who participated in 20-minute cold water immersions reported significant decreases in tension, anger, and depression, alongside increases in vigour and self-esteem.
The Wim Hof Studies
Wim Hof — the Dutch extreme athlete who popularised ice baths and cold exposure techniques — has been the subject of a fair amount of research, and it is worth being precise about what it found. A 2024 randomised controlled trial in Comprehensive Psychoneuroendocrinology put 84 midlife women with high stress and high depressive symptoms into either the Wim Hof Method or a comparison group doing slow-paced breathing and warm showers. Both groups improved — depressive symptoms down 24%, anxiety down 27%, perceived stress down 20% — and the improvements held at three months. The one thing that separated the Wim Hof group was less rumination after stress. So: something helped, but the cold was not obviously the active ingredient.
At Radboud University, Matthijs Kox and colleagues found that people trained in the method showed a blunted inflammatory response when injected with a bacterial endotoxin. That is a real and striking result about the immune system under a specific laboratory challenge — not evidence that cold showers stop you catching things.
It is worth noting that the research is not uniformly positive — some studies have found limited cardiovascular benefits, and cold exposure carries risks for people with certain heart conditions. But the mental health evidence is increasingly hard to dismiss.
Cold water swimming is not suitable for everyone. If you have a heart condition, Raynaud's syndrome, or other cardiovascular issues, consult your GP before starting. Never swim alone in cold water, and always acclimatise gradually — the cold water shock response is real and can be dangerous.
Why Cold Water Builds Friendships Faster Than Almost Anything

They watch out for signs of hypothermia. They bring hot drinks for the group. This mutual dependence — the knowledge that everyone is looking out for everyone else — accelerates trust in a way that pub conversations simply cannot.

How to Start: A Practical Guide
If you are reading this from under a blanket, the idea of voluntarily getting into cold water might seem absurd. Here is how to begin without hating every second of it.
Start in summer. Even Devon's rivers are a shock in July, but they are survivable. Get comfortable with outdoor water first, then extend the season gradually.
End your showers cold. Thirty seconds of cold water at the end of a warm shower begins the acclimatisation process. It is also an excellent way to practise the breathing control you will need in open water.
Go with others. This is the most important piece of advice. Find a group, join a session, ask a friend. Cold water swimming alone is both less safe and less enjoyable. The social element is not a bonus — it is the point.
Keep it short at first. Five minutes in cold water is plenty when you are starting out. The benefits come from the cold shock and the immediate aftermath, not from endurance.
Warm up properly afterwards. The "afterdrop" — when your core temperature continues to fall after you get out — is real and can catch you off guard. Have warm, dry clothes ready, a hot drink, and somewhere sheltered to change. Many swimmers swear by a flask of soup or hot chocolate and a good ten minutes of standing around talking before driving home.
